What is a care manager?

Care managers can fill an ongoing health role, and they can also be hired to assist in times of crisis should you require a full time point of contact to coordinate and monitor your care needs. They also can serve as an advocate for patients and families to identify and develop an individualized careplan to navigate the aging process or change in medical/physical condition.

Assessment

Due to our clinical expertise and experience in healthcare, we can provide a comprehensive assessment to determine the current care needs of you and your family. Based on comprehensive assessment and communication with family/friend members, we will be able to address short term and long term care needs.

Care Plan

Review of assessment results and discuss recommendations that will address the caregiver and care recipient’s short and long term needs. Updates and modifications will be reviewed and discussed as needed.

Our Care partners include and not limited to Physicians, Physician Specialist, Financial Advisor, Estate Planning, Insurance Advisors  (Understanding your benefits with Healthcare- Medicare, Medicaid, Managed Care), Home Care/Personal Care Professionals and Community Partners.

Senior Placement Advisor

Rs2Share recognizes that aging in place is the ultimate goal for most aging adults; however unexpected incidents and accidents happen that may warrant additional support and level of care in the home or an alternative home/setting. It may mean they remain at home and for others, who are not safe in the home setting, or those who are discharging from the hospital and/or a skilled nursing facility, they will need placement in an Independent/Residential Living Facility (ILF) Assisted Living Facility (ALF) Memory Care or Respite based on their care needs.
 
Rs2Share serves as your Senior Placement Advisor and is here to provide you insider knowledge of senior communities in the local area and assist with finding a new home or living arrangements.
 
Rs2Share researches, identifies and provides the client with a list of quality facilities based on family/caregiver/client specifications that will best fit client needs and criteria as they continue aging in place . Rs2Share’s goal is to make this a smooth and effortless process to put your mind at ease during this time of transition.
 
This is a free service to the client and includes:
  • Accompanying the client/family on tours
  • Coordinate the initiation of the paperwork process
  • Follow up with the facility assessment for their determined level of care
  • Provide follow up consultation after placement from a customer service perspective.
