What is a care manager?
Care managers can fill an ongoing health role, and they can also be hired to assist in times of crisis should you require a full time point of contact to coordinate and monitor your care needs. They also can serve as an advocate for patients and families to identify and develop an individualized careplan to navigate the aging process or change in medical/physical condition.
family support & Education
We will be a resource for researching and answering frequently asked questions and concerns regarding the stages of aging. We will also serve as mediator for care recipients and family, provide guides and options for caregiver self care = ‘Care for the Caregiver’

Due to our clinical expertise and experience in healthcare, we can provide a comprehensive assessment to determine the current care needs of you and your family. Based on comprehensive assessment and communication with family/friend members, we will be able to address short term and long term care needs.

Our Care partners include and not limited to Physicians, Physician Specialist, Financial Advisor, Estate Planning, Insurance Advisors (Understanding your benefits with Healthcare- Medicare, Medicaid, Managed Care), Home Care/Personal Care Professionals and Community Partners.
Senior Placement Advisor
- Accompanying the client/family on tours
- Coordinate the initiation of the paperwork process
- Follow up with the facility assessment for their determined level of care
- Provide follow up consultation after placement from a customer service perspective.
